CPHQ CE Program Approval

Do you have an educational program that would enhance the knowledge of healthcare quality professionals?

To offer CPHQ continuing education (CE) credit for the program, follow the steps below to apply.

Application Information

NAHQ offers CPHQ CE approval for educational programs, including but not limited to courses, conferences, workshops, and webinars.

60 minutes of presentation time = 1 CE credit

To apply for CPHQ CE approval for your program, fill out the application and submit it along with the application fee at least four weeks prior to the program date. NAHQ will not award CPHQ CE hours retroactively.

Application Fee

The price of the application fee depends on whether your organization is nonprofit or for profit and how many times you plan to offer the program in a one-year period:

*Applications submitted less than four weeks prior to the program date are subject to a $30 late fee.

The application fee is not refunded if an application is deferred or denied.

How to Apply

  1. Fill out the application and upload the following supporting files:
    1. CPHQ CE Program Outline (Outline also in document format) using the current CPHQ Content Outline
    2. Biographical Data Form for each presenter
    3. Marketing Materials
  2. Immediately after submitting your application, you will be redirected to the payment page on NAHQ's secure site.

  3. Select the appropriate fee (and late fee, if applicable), and proceed to submit payment with a credit card. You will receive a receipt of your payment via e-mail.

  4. Once your application has been reviewed, a letter of approval or denial will be e-mailed to you within two weeks from date of submission.

Approval Policy

If approved, your program will have approval for 12 months from the date of approval.

Program sponsors are responsible for monitoring attendance and providing each participant with evidence of attendance so they may receive credit for the program. Participants of that activity must self-report the CE they earned into their current recertification cycle.

We require that you upload a list of attendees within 30 days of the program date.

To accomplish this, you must:

  1. Complete this Attendee List Template
  2. Upload your Attendance List
