SUMMARY:Live Learning Lab Part 1 - Preventing Harm Before It Happens With Proactive Patient Safety Tools
DESCRIPTION:Speakers: \nSherri Hobbs-Messick MSM\, MSN\, RN\, CPHQ\, CPPS \nChief Quality\, Safety and Experience Officer – University of Maryland Medical Center \nMaggie Kane\, RN \nSenior Director of Patient Safety  – University of Maryland Medical Center \nSession Description: \nPatient safety starts long before an event occurs. In part one of a three-part Learning Lab series\, participants will learn how to spot risk early by using proactive tools and multiple safety signals to identify where harm is most likely to occur. Attendees will gain practical ways to recognize subtle warning signs\, including operational drift and normalization of deviance\, that often go unnoticed in daily work. The Learning Lab will equip participants with clear\, actionable approaches to strengthen early warning systems and prioritize risk before it escalates. This Learning Lab sets the stage for how to respond when safety events do occur in the next part of the series. \nLearning Objectives: \n\nApply proactive risk identification tools\, including FMEA‑style thinking and hazard analysis\, to recognize where patient harm is most likely to occur before an event happens.\nUse multiple safety signals\, such as culture data\, rounding insights\, and event reports\, to strengthen early warning systems and support timely decision making.\nRecognize signs of drift and normalization of deviance in everyday work and take action to strengthen operational resilience before harm occurs.\n\nRegister for part two of this Learning Lab series: here\n \nRegistration for part three of this Learning Lab series coming soon.

SUMMARY:Survey Survival Tips in 2026 – A Dive into the Changes and How They Impact the Survey Proc
DESCRIPTION:This presentation will provide an overview of the new infection prevention standards (The Joint Commission) and the crosswalk with CMS conditions of participation from the perspective of a former Joint Commission Surveyor. Our speaker will discuss the implications of the infection control chapter rewrite (TJC) as it relates to the survey process and provide solutions for mitigation. A question-and-answer session will be provided.

SUMMARY:Learning Lab Part 2 – Making Sense of Safety Events: From Reporting to Action
DESCRIPTION:Speaker/s: \nCarla Snyder\, MHA\, MT(ASCP)SBB\nExecutive Director \nNebraska Coalition for Patient Safety \nDescription: \nOnce a safety event or near miss occurs\, the real challenge becomes making sense of what happened and deciding what to do next. This part two of the three-part Learning Lab series focuses on how to evaluate event reports\, conduct effective reviews\, and uncover meaningful insights without bias. Participants will learn how to improve the quality of safety information\, make stronger triage decisions\, and translate findings into actions that actually reduce risk. The emphasis is on moving beyond surface level fixes to deeper learning that reflects how work truly happens. This session builds on part one and prepares participants to carry insights forward into sustained improvement and communication. \nRegistration for part three of this Learning Lab series coming soon. If you missed part one of the Learning Lab series\, you can access the on-demand version. \nLearning Objectives:  \n\nEvaluate patient safety event reports for clarity\, consistency\, and usefulness to support effective triage\, escalation\, and review decisions. \nConduct interviews and learning reviews that reduce hindsight bias and focus on understanding how work actually occurred at the time of the event. \nTranslate investigation findings into meaningful\, risk-reducing actions rather than weak or one-time fixes. \nExplain how the tools discussed (SAC Matrix\, Interviewing Tips\, Cause Mapping and Causal Statements\, Strength of Interventions\, etc.) can be used to improve your analysis and response to reported events.
