Performance Improvement Program: The Five Essentials

In this interactive session, participants will be offered the opportunity to gain new knowledge, strategies, and tools to adopt in the development of an impactful performance improvement program. Applying a structured and comprehensive approach can ease the burden in addressing the healthcare quality challenges within our complex and fragmented healthcare system. The ColumbiaDoctors Performance Improvement Program Matrix will feature the itemized application of the "Performance Improvement Program: The Five Essentials" in decreasing ICU length of stay, reducing hospital acquired infections and lastly, taking on the opioid crisis in the physician practices. The five essentials are: 1). champion 2). collaboration 3). tool 4). analysis 5). model. This session will also feature "Dotmocracy", whereby participants will work with their colleagues to discuss and critique which example under each of the five PI essentials discussed will be the most feasible to adopt in their own organizations.

Ana Esteban is the Performance Improvement (PI) Specialist for ColumbiaDoctors-The Faculty Practice Organization of Columbia University Irving Medical Center. In this role, she is responsible for patient-centered quality improvement initiatives geared to promote excellence in the 1,800 provider multi-specialty ambulatory care practice. With her guidance, the quality division adopted an effective PI model based on the: Plan-Do-Study-Act (PDSA)/Inspiration-Infrastructure-Education-Evidence (I2E2)/Sort-Set in Order-Shine-Standardize-Sustain-Safety (LEAN 6S) models. This has since evolved into the ColumbiaDoctors Performance Improvement Program featuring the five essentials: champion, collaboration, tool, analysis, and model. Prior to joining ColumbiaDoctors, Esteban championed PI collaborations with healthcare quality organizations such as the Institute for Healthcare Improvement, Healthcare Environment and the Johns Hopkins Armstrong Institute for Patient Safety and Quality. These resulted in impactful initiatives on: early progressive mobility to decrease ICU length of stay and reducing hospital acquired infections-VAEs, CLABSIs and CAUTIs in the ICUs. She is a Certified Professional in Healthcare Quality® (CPHQ).
