Danielle Phelps Swartz serves as Vice President of Population Health and Value-Based Care at Cone Health, where she provides executive leadership for the organization's population health strategy, value-based care portfolio, and risk-based transformation efforts across Medicare, Medicaid, Commercial, and employer-sponsored populations. 

A visionary healthcare executive with deep expertise in value-based care, population health management, payer-provider partnerships, and healthcare transformation, Danielle leads enterprise strategies that improve clinical outcomes, enhance patient and provider experiences, and optimize the total cost of care. She is responsible for advancing Cone Health's value agenda through the integration of care delivery, data analytics, digital innovation, and performance improvement initiatives that drive sustainable results across diverse risk arrangements. 

Danielle has extensive experience developing and scaling value-based care models, clinically integrated networks, payer collaborations, and direct-to-employer strategies. Her leadership has been instrumental in aligning physicians, health system operations, and community partners around population health objectives while accelerating organizational capabilities in care management, quality performance, risk management, and health equity. Recognized for her ability to bridge strategy and execution, she has contributed to value-based care initiatives at both state and national levels and is known for building high-performing teams that deliver measurable impact. 

As a trusted leader and advisor, Danielle is passionate about shaping the future of healthcare through innovation, strategic partnerships, and leadership development. She serves on the boards of Cone Health Ventures and HealthTeam Advantage, helping guide growth, innovation, and value creation across the healthcare ecosystem. She has also served as an Advisor to the Women's Leadership Program at the University of Tennessee, Knoxville, supporting the development of future healthcare leaders. 

Danielle holds a Master of Healthcare Administration from Walden University and a Bachelor of Science in Nursing from Duquesne University.
