1. Introduction. 

The National Association for Healthcare Quality (“NAHQ,” “we,” “our,” or “us”) respects your privacy. This policy describes the types of information we may collect about you from our websites, microsites, and through other electronic messages and means, including email. This policy also describes our practices for using, maintaining, protecting, and disclosing that information. If you do not agree with our policy and practices, then your choice is not to use our websites or correspond with us through electronic communications.  

This policy may change from time to time. Your continued use of our websites after we make changes is acceptance of those changes, so please check the policy periodically for updates. 

2. Information We Collect About You and How We Collect It.

We collect several types of information from and about users, including information: 

  • By which you may be personally identified, such as name, postal address, email, telephone numbers, social media handles, and any other identifier by which you may be contacted online or offline (“personal information”); 
  • That is about you, but individually does not identify you, such as survey responses and demographic information (e.g., gender, birth date, certifications and credentials earned, highest degree earned, primary position, leadership positions, and years of experience);  
  • About your payment, such as credit card information; and 
  • About your internet connection, the equipment you use to access our websites and usage details. 

We collect this information: 

  • Directly from you when you provide it to us; 
  • Automatically as you navigate through our websites; 
  • From third parties (e.g., your employer, sponsor, or our contractors); and 
  • Via publicly available sources (e.g., Facebook and LinkedIn).  

As you navigate through and interact with our websites, we may use automatic data collection technologies to collect certain information about your equipment, browsing actions, and patterns, including details of your visits to our websites and information about your computer and internet connection, including your IP address. The information we automatically collect is statistical data and may include personal information or we may maintain it or associate it with personal information we collect in other ways or receive from third parties.  

The technologies we use for this automatic data collection may include: 

  • Cookies. A cookie is a small file placed on your computer. You may refuse to accept cookies by activating the appropriate setting on your browser. However, if you select this setting, you may be unable to access certain parts of our websites. Unless you adjust your browser settings to refuse cookies, our system may issue cookies when you access our websites. 
  • Web Beacons. Pages of our websites and our emails may contain small electronic files known as web beacons (also referred to as clear gifs, pixel tags, and single-pixel gifs) that permit us to count users who have visited those pages or opened an email and for other related website statistics (for example, recording the popularity of certain website content and verifying system and server integrity). 
  • Third-Party Tracking. Google Analytics, data analytics firms, and others may track and analyze your visits to our websites and social media pages. This enables us to improve our services and to target ads that may be of interest to you, including across the internet and via email to you.  

3. Information You Provide to Us. 

The information we collect from you may include: 

  • Information you provide by filling in forms online. This includes information provided at the time of registering to use our websites, using our services, posting material, or requesting further services or information;  
  • Records and copies of your correspondence (including email addresses), if you contact us; 
  • Your responses to surveys; 
  • Details of transactions you carry out through our websites and of the fulfillment of your orders; you may be required to provide financial information before placing an order through our websites; and/or 
  • Your search queries on our websites. 

You also may provide information to be published or displayed on our websites or transmitted to other users of the websites or third parties (collectively, “User Contributions”). Your User Contributions are posted on and transmitted to others at your own risk. Although we may limit access to certain pages, please be aware that no security measures are perfect or impenetrable. Additionally, we cannot control the actions of other users of the websites with whom you may choose to share your User Contributions. Therefore, we cannot and do not guarantee your User Contributions will not be viewed by unauthorized persons. 

4. Accessing and Correcting Your Information.

If you have an account with us, including because you are a NAHQ member, you may review and correct certain parts of your personal information by logging into our website and visiting your account page. Whether or not you have an account with us, you may send us an email to request changes to your personal information. However, we may not delete your personal information. Also, we may not make a requested change if we believe the change would violate a law or legal requirement, cause the information to be incorrect, or not be in the best interest of NAHQ. 

NAHQ also may retain certain information about you in aggregated form. Once aggregated, this information does not directly or indirectly reveal your identity. This privacy policy does not restrict our use of aggregated data. Aggregated data will not be subject to access or correction requests. However, if we combine or connect aggregated data with personal data so it can directly or indirectly identify you, we treat the combined data as personal data that is subject to this privacy policy.

5. How We Use Your Information.

We use information we collect about you or that you provide us, including any personal information: 

  • To present our websites and their contents to you; 
  • To provide you with information, products, or services you request from us; 
  • To facilitate your participation in in-person and online courses, meetings, and other events; 
  • To provide you with notices about your membership, certification, or other credential, including expiration and renewal notices; 
  • To carry out our obligations and enforce our rights arising from any contracts entered into between you and us, including for billing and collection; 
  • To notify you about changes to our websites or any products or services we offer or provide through them; 
  • To allow you to participate in interactive features on our websites; 
  • To fulfill any other purpose for which you, your employer, or another organization with which you are associated provide it; 
  • In any other way we may describe when you provide the information; 
  • For any purpose consistent with our exempt purpose; and  
  • For any other purpose with your consent.

In addition, we may use aggregated information about our users, including you, and information that does not identify any individual, without restriction. 

6. Disclosure of Your Information.

We may disclose personal information that we collect, or you provide, as described in this privacy policy: 

  • To our employees; 
  • To contractors, service providers, and other third parties we use to support our business; 
  • To a successor in the event of a merger, reorganization, or similar occurrence in which some or all of NAHQ’s assets, including personal information held by NAHQ, is among the assets transferred; 
  • To publicly identify you as having achieved certification as a Certified Professional in Healthcare Quality or attained another credential; 
  • To third parties to market products or services to you, including by way of targeted ads; 
  • To third parties for workforce development purposes; 
  • To third parties in NAHQ’s discretion, consistent with its exempt purpose;  
  • To fulfill the purpose for which you, your employer, or other organization with which you are associated provide it; 
  • For any other purpose disclosed by us when you provide the information; and 
  • With your consent. 

We may also disclose your personal information: 

  • To enforce or apply our terms of use and other agreements, including for billing and collection purposes;  
  • To comply with any court order, law or legal process, including to respond to any government or regulatory request; and  
  • If we believe disclosure is necessary or appropriate to protect the rights, property, or safety of NAHQ, our members, or others. This includes exchanging information with other companies and organizations for the purpose of fraud protection and credit risk reduction. 

We may disclose aggregated information about our users, including you, and information that does not identify any individual, without restriction. 

7. Choices About How We Use and Disclose Your Information. 

You have certain choices regarding your personal information: 

  • Tracking Technologies and Retargeting. You can set your browser to refuse all or some cookies, or to alert you when cookies are being sent. If you disable or refuse cookies, please note that some parts of our websites may be inaccessible or not function properly. You can disable Google Analytics by following the instructions available here. You also have choices regarding the collection and use of information for ad targeting, as explained here. 
  • Promotional Offers from NAHQ and Others. If you do not wish to have your personal information used by NAHQ or third-parties to promote our own or third-parties’ products or services, you can opt-out by sending us an email stating your request to info@nahq.org. If we sent you a promotional email, you may unsubscribe to future promotional emails by following the options presented to you in the email’s footer.  
  • Social Media Platforms and Networks. We encourage you to review your privacy options and settings with the social media platforms and networks you use to understand what choices you have about sharing information from those platforms and networks with us. 

8. California Privacy Requests. 

California Civil Code Section § 1798.83 permits users of certain websites that are California residents to request certain information regarding the disclosure of their personal information to third-parties for their direct marketing purposes in the preceding calendar year. To make such a request, please contact us. 

9. Children Under the Age of 18. 

Our websites are not intended for children under 18 years of age. No one under age 18 may provide any information to or on our websites. We do not knowingly collect personal information from children under 18. If you are under 18, do not use or provide any information on the websites or on or through any of its features, register on the websites, make any purchases through the websites, use any of the interactive or public comment features of the websites, or provide any information about yourself to us, including your name, address, telephone numbers, email address or your screen name or username. If we learn we have collected or received personal information from a child under 18 without verification of parental consent, we will delete that information. If you believe we might have any information from or about a child under 18, please refer to the information provided in the Contact Information section below and contact us. 

10. Data Security.  

We use measures that are appropriate to protect your personal information from improper access, use, alteration, and disclosure. All information you provide us is stored on secure servers behind firewalls. Any payment transactions and login information is encrypted using SSL technology. 

The safety and security of your information also depends on you. If you have a password to access certain parts of our websites, you are responsible for keeping this password confidential. We ask you not to share your password with anyone. 

The transmission of information via the internet is not completely secure. Although we take steps to protect your personal information, we cannot guarantee the security of your personal information transmitted to us electronically, including through our websites. Any transmission of personal information is at your own risk. We are not responsible for circumvention of any privacy settings or security measures contained on the websites. 

11. Changes to our Privacy Policy. 

If we make material changes to how we treat our users’ personal data, we may notify individuals with NAHQ accounts, including NAHQ members, by email to the primary email associated with their accounts. Individuals with NAHQ accounts are responsible for ensuring we have up-to-date active and deliverable email addresses for them. The date this privacy policy was last revised is identified at the top of this page. You are responsible for periodically visiting our websites and this privacy policy to check for any changes.
