Participants will understand the importance of clinical pathways as safety nets to ensure optimal care is provided to all patients. Presenters will share details of how to build an integrated, team-based approach to care that focuses on the patient versus working in silos. A discussion of how this approach was successfully implemented in both large tertiary facilities and small rural facilities will also be included.
