My co-authors and I lead a successful safety initiative within the Duke Health Integrated Practice called the Patient Safety Liaison Program. This program empowers front-line staff to take patient safety concerns into their own hands and lead efforts to improve performance and reduce the risk of harm. This program has been successful in lowering our incidence of patient harm while increasing our number of safety reports and good catches.
