An Integrated Approach to Decreasing Unnecessary Hospital Stays

The Transitional Care Service (TCS) is a physician-managed initiative designed that addresses the rise of increasing use of hospital beds for care that could be provided in an ambulatory setting. This service bridges the gap between inpatient and outpatient care by facilitating safe, timely discharges for patients. We will discuss how this service enables emergency and hospital providers to expedite patient transitions by arranging insurance pre-authorization and scheduling of outpatient testing, and by arranging expedited follow-up appointments. By enhancing care coordination and reducing lengths of stay, the TCS has decreased unnecessary hospital days, thereby optimizing the use of hospital resources.
