Screening for Social Drivers in a Community Health Center: Enhancing Patient Outcomes’
Our project aims to connect patients to essential services by screening for Social Drivers of Health (SDOH) at every visit type. Screening tools and a Care Management platform are integrated into our EMR, enabling us to refer patients to community-based organizations. Automatic referrals are made within the EMR for any positive answers. Partnering with FindHelp.org provided an additional way to connect patients to resources. In 2024, we screened 40,261 of 49,000 patients, with 17,560 (44%) showing positive responses. Of those, 8,336 were seen by Care Management, and 4,162 accessed services via FindHelp. A survey showed 73% found FindHelp useful.
