Evolution of a Safety Huddle Through Continuous Improvement
Medical errors are the 3rd leading cause of death in the United States.(1) Patient safety reporting systems tend to underreport events.(2,3) Safety huddles can augment timely communication of safety events.(4,5) Our health system established executive safety huddle in 2023. To meet this objective, our service line operationalized a framework across 14 locations and >1000 staff. Strategies included daily safety huddles, virtual communication channels, and frontline huddles. We will explore our safety huddle framework including how we leveraged Just Culture to promote psychological safety, barriers and lessons learned. Case studies will show how the huddle structure addresses and mitigates safety concerns.
