Leveraging Nurse-Led Telehealth and Continuous Glucose Monitoring to Improve Diabetes Outcomes
This project highlights an innovative, nurse-led telehealth-based diabetes management program designed to improve glycemic control and reduce hospital utilization for patients with poorly controlled diabetes (A1c >8%). Using continuous glucose monitoring (CGM) technology and structured transitional care, this initiative enhanced care coordination, improved patient engagement, and reduced disparities in diabetes care. Through a structured Plan-Do-Study-Act(PDSA) approach, the program demonstrated reductions in A1c and healthcare utilization, particularly among high risk populations. This session will provide actionable insights into implementing a nurse-driven telehealth intervention, leveraging health data analytics, and sustaining process improvements in diabetes care management.
