From Reporting to Learning: Building a Continuous Feedback Culture
Effective event reporting is an integral part of a home health agency's safety culture, serving as both a risk management tool and catalyst for continuous improvement. This session explores strategies to transform event reporting from a reactive process into a proactive tool for meaningful change. Attendees will learn how to analyze events with a focus on learning rather than blame, close communication loops to ensure follow-up, and engage in psychologically safe conversations that encourage staff participation. By fostering transparency and continuous feedback, organizations can strengthen patient and staff safety while driving lasting improvements.
Tool: SAFER: A Framework for Event Reporting & Continuous Feedback
Problem: Refine the event reporting process that (1) supports an environment where reporting is psychologically safe, (2) analyzes events for opportunities and learnings, and (3) facilitates open conversation for event feedback and follow-up.
Tool Selection: The SAFER framework addressed common barriers to event reporting, including underreporting, fear of blame, lack of follow-up, and missed learning opportunities.
Usage: We applied the SAFER framework to our event reporting process by supporting a psychologically safe reporting environment, analyzing data for opportunities, facilitating open conversation and problem-solving, engaging staff in meaningful feedback and follow-up, and reinforcing continuous improvement through action.
Results: The PDCA tool was used to identify barriers in the event reporting process, leading to the implementation of the SAFER framework. This resulted in increased event reporting, improved event analysis methods, and enhanced event follow-up. Barriers included time constraints, competing priorities, and workflow challenges.
