Patient Safety Indicator (PSI) Reduction Through Implementation of a Performance Feedback Loop

Patient Safety Indicators (PSIs), developed by AHRQ, identify potentially preventable hospital safety-related adverse events. UC Davis Health (UCDH) collaborates with Vizient to enhance patient safety, reduce costs, and improve care quality. Recognizing PSI's impact, UCDH's Quality and Safety Department implemented multiple PDSA cycles to refine workflows. The second cycle's enhanced review process, including a physician champion and second-level reviewer, significantly reduced PSI events. The third cycle focuses on physician education to drive further improvements. Through iterative cycles, UCDH has strengthened its PSI review process, improved patient safety, and engaged key stakeholders to reduce adverse events and enhance care quality.
