The Age‑Friendly Accelerator: Leveraging Data, Governance, and EHR to Drive Change Systemwide

Corewell Health launched a systemwide Age‑Friendly care transformation to prepare for the 2026 CMS Structural Measure and improve outcomes for older adults. Using a structured performance‑improvement approach, the team identified care gaps and implemented consistent, systemwide practices aligned with patient goals, medication safety, frailty identification, social vulnerability screening, and leadership engagement. Embedding Age‑Friendly elements into the EHR improved reliability, reduced variation, and strengthened care transitions. Early results demonstrate increased screening consistency, more documented “What Matters Most” conversations, and enhanced interdisciplinary collaboration. These efforts support CMS readiness while advancing person‑centered, data‑driven, high‑reliability care for older adults across the health system.
