Leveraging Clinical Decision Support and Pharmacy Workflows to Enhance PSI-12 Patient Outcomes
This presentation describes a multidisciplinary, system-level initiative at OU Health to reduce hospital-acquired PSI-12 events by improving the reliability of pharmacologic VTE prophylaxis. Leveraging executive sponsorship, standardized order sets, enhanced EHR clinical decision support, and daily pharmacy-led audits, the team addressed gaps in ordering and administration. Results demonstrate sustained year-over-year improvement in PSI-12 performance, highlighting how embedded workflows and real-time escalation can reduce preventable harm and support patient safety, quality outcomes, and value-based performance.
