Improving Perinatal Care: Leveraging Simulation and Accountability to Reduce Severe PPH Events
Following several severe postpartum hemorrhage (PPH) events, a multidisciplinary Taskforce launched a performance improvement initiative to reduce severe maternal morbidity. Using Lean and Six Sigma methods, a Cause‑and‑Effect analysis identified inconsistent use of the PPH protocol due to knowledge gaps and lack of accountability. The team implemented targeted PPH drills and simulations and established an OB Leadership group to review every PPH case and provide direct feedback to clinical teams. Within one year, unplanned hysterectomies decreased by 87% and ICU admissions by 43%, demonstrating significant improvement in maternal safety and reliability of perinatal care.
