No Appointment Left Behind: Improving 30‑Day Psychiatric Follow‑Up Rates
This quality improvement initiative improved 30‑day follow‑up appointment rates for Medicare patients by standardizing discharge scheduling. A multidisciplinary team conducted a gap analysis, identifying workflow variation, communication issues, and inconsistent provider practices. Baseline follow‑up performance averaged 63.6%. The intervention added a required pre‑discharge appointment, updated EHR documentation, and daily huddles to enhance communication. Obstacles included weekend discharges and documentation variability, addressed through expanded targeted training to weekend staff and reinforcing documentation standards. Follow‑up completion increased to 80%, surpassing state and national benchmarks and reducing process variation while strengthening care continuity.
