Reliable Prevention Starts Here: The Skin Integrity Huddle Tool
This Best Practice Tool demonstrates how a multidisciplinary team reduced pressure injuries by implementing a risk‑triggered Skin Integrity Huddle supported by a 14‑question prevention checklist and integrated safety event reporting, real‑time process monitoring, and post‑event learning. The tool standardizes prevention behaviors, accelerates early risk identification, and connects bedside practice to measurable improvements. Results include a reduction in total HAPIs from 46 to 5 and elimination of reportable HAPIs. This approach is scalable, actionable, and aligned with NAHQ’s competency domains in performance improvement, safety, and quality review.
