Collaborative Power: Engaging in Quality Improvement Across Settings

We often hear we need to look outside the traditional “four walls” when it comes to improving quality of care. However, engaging outside of our own organizations can be a challenging proposition. Learn how to engage cross-sector stakeholders and run collaborative improvement efforts to support population health and care transitions improvements with a broader set of stakeholders. This session will share tactics and approaches through real-world examples to help the audience develop their own toolbox from garnering internal and external buy-in to engaging in successful data-driven quality improvement across healthcare sectors and the healthcare continuum.
