Redesigning Medical Center Operations to Drive Quality, Leadership, and Continuous Improvement

VISN2 implemented an ISO 9001 certified Quality Management System (QMS) across Sterile Processing Services (SPS) at over 10 hospitals throughout the network, standardizing processes to enhance patient safety, reduce risk, and improve operations. The QMS, which includes an electronic Quality Management System (eQMS), provides standardized reprocessing instructions for reusable medical devices, ensuring consistent, data-driven workflows. By minimizing human error, improving process reliability, and promoting a culture of safety, the QMS drives quality improvement and better patient outcomes. This initiative exemplifies leadership in quality management, clinical governance, and regulatory compliance, fostering continuous improvement and operational excellence for veterans.

Problem: VHA's enterprise-wide assessment revealed that Sterile Processing staff failed to consistently follow Manufacturers' Instructions for Use, risking infection for thousands of veterans. Previous corrective actions were unsustainable, prompting VISN2 to implement standardized processes, enhance staff competence, and modernize practices to ensure consistent, safe care for veterans.

Measurement: Improper reprocessing of critical equipment lacked consistent measurement, prompting media attention and congressional hearings. VHA conducted compliance audits to validate inconsistencies in sterile reprocessing processes, leading to the establishment of new standards and metrics to ensure consistent, safe practices.Analysis:VISN2 conducted direct observations, reviewed documentation, SOPs, and internal audits to assess compliance with Manufacturers' Instructions for Use (MIFUs). However, the lack of document control led to variations in the completeness and accuracy of reprocessing practices, highlighting the need for standardized procedures and improved oversight.

Implementation: VISN2 redesigned sterile processing across 10 medical centers, emphasizing document standardization, education, leadership and stakeholder buy-in, and risk reduction. Advanced technology, including dashboards and quality objectives, improved process visibility, accountability, and efficiency. Change management principles were applied to minimize resistance, enhancing patient safety and operational performance.

Results/Discussion: Critical to success and sustained ISO certification were management reviews, which ensured ongoing evaluation, sustainment, and improvement of processes. Creating the ISO Governing Council provided oversight of policy, vision, and objectives. Additionally, leveraging Microsoft Power Suite to create dashboards and automate document control streamlined operations, improved compliance, and enhanced efficiency.
