Reducing Patient Falls and C. Diff Using Lean Huddle Boards
This initiative leveraged Lean Daily Management Huddle Boards and High-Reliability Organization principles to reduce patient falls and eliminate C. difficile (C. diff) infections in a surgical orthopedic unit. In November 2023, a redesigned board with a Just One Thing focus was implemented, promoting targeted daily interventions. Results included a 50% reduction in falls, a 100% decrease in major fall-related injuries, and zero C. diff infections. Increased staff engagement and patient satisfaction reinforced sustainability. This session will explore how structured huddles, real-time data tracking, and leadership alignment can drive measurable improvements in patient safety and quality outcomes.
Tool: Huddle boards are visual management tools used in healthcare to track key metrics, enhance communication, and drive daily problem-solving. High-Reliability Organization (HRO) principles such as preoccupation with failure and deference to expertise promote proactive risk management, ensuring consistent, data-driven interventions that improve patient safety, quality outcomes, and staff accountability.
Problem: This initiative targeted patient falls and Clostridioides difficile (C. diff) infections, addressing inconsistent communication, lack of real-time data, and insufficient accountability. The team improved safety measures, standardized prevention strategies, and fostered proactive problem-solving.
