Engaging Patients and Families for Medication Safety

Patients and their families are the only constants in the increasingly complex health care systems. A patient may receive care from different facilities and multiple care providers from different disciplines. Every point in the care process contains some inherent risks. Adverse drug events or harm may result from problems in practice, or with products, procedures, or systems, which are not usually the fault of the health care provider.

Engaging patients and families who have experienced harm can provide insights and learning concerning system failures. This webinar will cover creating opportunities for patients and families to reduce medication-related harm; empowering patients and families for medication safety, particularly through WHO Patient Engagement Tool: “5 Moments for Medication Safety”; patients, families and health workers partnering for medication safety; and developing programs for patient and family engagement -country experience, followed by questions and answers.
